If your driver’s license is lost, stolen, or damaged/destroyed, and you have a valid Medical Examiner’s Certificate (MEC), you can get a replacement license, known as a duplicate.
If your name or address has changed since you got your last license, there are additional steps to update your address on your new license.
Follow these steps to get a replacement CDL
Step 1: Print and complete the required form
- Complete an application for duplicate of current driver's license or permit (Form 1-B).
Step 2: Make an appointment
- When you replace a driver’s license, please make an appointment at a DMV office.
Step 3: Get your replacement CDL in the mail
- You’ll get a temporary license at your appointment, and your permanent driver’s license replacement arrives through the mail.
- Your new license is only mailed to the mailing address on file with the DMV. If you have mail forwarding, you will NOT receive your new driver’s license.
