Filing A Request for Grievance Arbitration Services with the State Board of Mediation & Arbitration: 

To request grievance arbitration services, employers or employee organizations must submit a demand for arbitration on a completed grievance arbitration request form (available below).  Employers or employee organizations may file in-person or by US Mail.  This form must be signed and include a general explanation of the dispute and a $200.00 filing fee.  

IMPORTANT NOTICE: THIS FORM IS ONLY FOR A DISPUTE THAT RELATES TO OR ARISES UNDER A COLLECTIVE BARGAINING AGREEMENT BETWEEN A UNION AND AN EMPLOYER.  Please contact the SBMA at (860) 263-6880 if you are uncertain whether it is appropriate for you to use this form.
