What forms or documents need to be submitted to obtain site approval?
Answer:
The completed Form DAS-053 with attachments, and the property appraisals.

I have multiple projects occurring at the same school site a CV code violation RR roof replacement and EC energy conservation each having a separate State Project Number. Does each of these projects require a separate Form DAS-053 submission?
Answer: No. This form is to be used for site acquisition grants only.

Will an incomplete or un-submitted Form DAS-053 impact my grant approval?
Answer:
Yes, a completed Form DAS-053 is required and must be submitted at the time of grant application in order to allow a grant commitment letter* to be issued.
*NOTE: Issuance of a grant commitment letter does not guarantee grant approval. Plan Review department review, approval and sign-off must be obtained to complete the grant process.

What type of acknowledgment will I receive from the Office of Grants Administration once a completed Form DAS-053 has been submitted?
Answer: Any issues discovered in submission documents will be addressed. The DAS will provide a notification of deficiency either through email correspondence or via a telephone call, followed up by a formal letter.

My school construction site is located on or near a waterway. How do I know what permits or approvals are required?
Answer:
- Initially, meet with local officials regarding a Storm Water Pollution Control Plan (SPCP), a DEEP Storm Water Discharge Permit, Flood Management Permit and approvals, local Inland Wetlands and Watercourses requirements, Health Department regulations, Zoning requirements and Building Permits.
