Overview

All officially issued revisions to a school building project made after Final Plan Review may affect reimbursement eligibility and must be submitted to DAS for review. Guidance is provided for the submission and review of addenda and other bid-phase revisions, construction change orders, and other change directives.

All school construction change orders or construction change directives must conform to the terms and conditions outlined in FORM SCG-3050 Bulletin of Common Change Order Ineligibilities. A complete change order submission consists of:

• A signed and dated FORM SCG-042.CO Notice of Change Order
• A completed FORM SCG-043.CO Change Order Summary sheet emailed to DAS.scg.changeorder@ct.gov
• All corresponding back up information for each trade contractor must be submitted electronically.
