Report Test Results

• Test Results Form (form 1020-C) for electric elevators, hydraulic elevators and escalators/moving walks. This form is for use by authorized elevator contractors only.

Record the Owner's Address / Change Address on Record

• Change of Owner/New Address  - This form is to be used for updates to owner registration and change of address information.  It can be mailed, emailed or fax into the office.  Please make sure your elevator registration number is included otherwise it will not be accepted.
