Documents required annually from current Lessors

  1. Insurance Certificate. Lessors are responsible for providing a current insurance certificate to DAS as required by Section 9.01 of the form lease agreement.
  2. Fire Drill and Safety Compliance Form. Form to be completed annually by Lessor. 
  3. OPM Form 1 Gift and Campaign Contribution Certification. Form to be completed annually by Lessor.  Please complete the required information on page 2. If no contributions were made, then please indicate as such by writing in the word, “none”. Your signature and acknowledgement before Commissioner of the Superior Court or Notary Public is required.

 

Documents Required to Purchase Surplus State-owned Real Estate

  1. Surplus Proposal Form.  Use this form to submit your proposal to purchase state-owned surplus real estate.
  2. Form Sale Agreement.  The State standard approved sale agreement.
  3. Contractor Authorization. Sample of a certificate or resolution that is acceptable to the Department of Administrative Services.  Please complete and sign the document as required.  Note: A certificate that is self-serving will not be accepted.
  4. OPM Form 1 Gift and Campaign Contribution Certification.  Please complete the required information on page 2. If no contributions were made, then please indicate as such by writing in the word “none.” Your signature and acknowledgement before the Commissioner of the Superior Court or a Notary Public is required.
