DAS Business Office's Item Disposal Request Guide for Customer Agencies

DAS and it's Business Office customer agencies can download and fill out the Item Disposal Request Form which is used by DAS/AM to begin the disposal process. Please complete the form to the best of your knowledge for each item, making sure to separate any inventory tagged items on separate rows.

Send completed forms to DAS/AM's group e-mail: DAS.AssetManagement@ct.gov. Digital excel forms are perfectly acceptable; there is no need to print and hand sign the form when submitted via e-mail.

If able during submittal, please include clear pictures of all items to expedite the process . Pictures are a requirement of the surplus systems that Asset Management enters the data into.

Note: Any unwanted data processing equipment, such as personal computers and laptops, should be returned to your respective information technology units for data deletion. The IT units can then send the form into DAS/AM if they choose not to repurpose the items.
