LifeCourse planning is built on the core belief that people and their families have the right to live, love, work, play, and pursue their life aspirations – whether or not they have a disability.

Personal profile

Planning works best when individuals and families are able to successfully communicate to others about what is most important in the person’s life. Using the One-Page Profile provides a way to know what really matters to the person and can be taken with them as they move through services and come into contact with different people. The Personal Profile helps others understand what to pay attention to and what works best to create positive change for the person.

Life stage trajectory

LifeCourse Planning helps families envision what matters most. The Charting the LifeCourse Experiences and Questions Booklet (PDF) can help families make sure that the right questions are asked on behalf of their loved one.

Using the LifeCourse Tools for Developing a Vision (PDF) form and the Life Stages Trajectory (PDF) form can help both the person and his or her loved ones to prioritize what is most important and to help guide decisions about current and future life experiences that will help move the person toward an inclusive, productive life in the future.

Share or customize lifecourse tools

You can use, share, and customize LifeCourse tools and materials. LifeCourse Nexus offers materials in quantity, and graphics if you need them. You are free to make changes to them and place them in your documents and materials, granted you give us credit.
