How do I know if I am eligible for services?


The State of Connecticut offers services and supports for individuals with intellectual disability. The Department of Developmental Services (DDS) runs these programs, supporting citizens throughout Connecticut.


Am I eligible for services?

To be eligible for these supports or services, a person must:


Apply for Services
  1. Print and complete Application (English) (PDF) or Print Application (Spanish) (PDF)
  2. Show that you are applying for services for an Intellectual Disability on page 2. (Not for the Autism Spectrum Disorder Program).
  3. Gather and submit all items listed on the Application Checklist (also listed on page 6 of the application):
    • Psychological and Educational Testing performed through the age of seventeen (17)
      • Psychological evaluations including cognitive and adaptive scores must be done prior to the age of eighteen (18)
    • Medical history and most recent physical
    • HIPAA acknowledgment form (also on page 6 of the application)
    • Guardianship or conservatorship forms if applicable
    • Proof of CT residence
    • Copies of the following:
      • Birth certificate
      • Social Security card
      • Medical insurance card
    • Educational information
  4. If you are MISSING any of the items listed, you’ll need to complete a Release of Information form (doc). (also on page 7 of the application)
  5. Submit all items to:
