I have had COVID-19 in the past 90 days. Do I need to submit documentation?

Yes, if you have been fully vaccinated or partially vaccinated, please submit your documents via the COVID Navigator App. If you have not had your vaccination(s) and will need to conduct weekly testing, you will not need to begin testing until the 90 days from onset of COVID-19 symptoms or a positive test result. You will need to submit a medical exemption form to confirm your COVID-19 positive status. The form will be valid for 90 days from onset of COVID-19 symptoms or positive test. You will need to begin weekly testing after the 90 days, if you are not fully vaccinated.

 

Note that as of 12/27/2021, due to current shortage of testing capability, home or self-testing with an FDA approved or authorized COVID-19 test kit is an acceptable testing method for a 90-day waiver.
