How do I prove that I have had COVID-19 in the past 90 days?

If you have been diagnosed with COVID-19 in the past 90 days, you have two options for information submission based on your circumstances:

  • If you received a positive COVID-19 test, you simply need to fill out the testing attestation form, and attach a copy of your test results. 
  • If you did not receive a COVID-19 test, but were a presumed positive based on a diagnosis from your health professional, your provider must full out a COVID-19 diagnosis attestation form and fill it out.

Please be reminded that when 90 days have passed, you must either continue to submit weekly test results or become fully vaccinated. Note that as of 12/27/2021, at-home tests are now considered a sufficient test for a 90-day waiver.

 

Revised Update: Note that as of 12/27/2021, due to current shortage of testing capability, home or self-testing with an FDA approved or authorized COVID-19 test kit is an acceptable testing method for a 90-day waiver.
