Submit Two Forms to the Bank:

 

The Participating Addendum (please supply link if possible)

The Participating Addendum is a three-page document that goes along with the State’s contract. This document includes the name and email of the person who will help set up your bank account for receiving ACH rebate payments.

The Know Your Customer Form (Please supply link if possible)

The Know Your Customer form is used to make sure you meet the Office of Foreign Assets Control federal requirement to open an account and provide your annual budget so the bank has an estimate of the line of credit you will need to start your program.
