Per the Connecticut General Statutes, DAS cannot comment on or investigate the liability of a decedent’s services or another state agency.
You must file a PC-212 with the local Probate Court, DSS or the appropriate state agency and inform them of your loved one’s passing. The Probate Court or appropriate state agency will then send the necessary documents to DAS, who will in turn initiate the appropriate action.
