DAS Fleet Operations requires the mileage of our vehicles to be reported monthly. You can report this as well as view your monthly vehicle charges, maintenance and other vehicle information in Chevin Fleetwave.

Per General Letter 115: In accordance with State Records Retention guidelines (FISCAL-068), agencies shall keep and store daily mileage logs (Handwritten Form CCP-40, or telematics generated CCP-40 trip logs) for each state-owned vehicle assigned to them, and daily operator assignments for pool vehicles.

Any vehicle with a telematics device installed may substitute the hand-written CCP-40 log with the electronic trip logs automatically captured by the device. Agencies must continue to keep a daily log of operator assignments for each vehicle and submit any reports requested by the Director of DAS Fleet Operations.
