State Police Criminal History Record Request Form - National:
Submit the Criminal History Record Request using the State Police digital system only AFTER your Taxi application is accepted by the DOT. Have your DOT Application Docket Number with you, as the Docket Number may be required. Once you have a Docket Number, email the Regulatory and Compliance team member working on your application and they will email you a code and a link to apply for a State and Federal Criminal History check. We will be able to view the results in a secure database that belongs to the Connecticut Department of Emergency Services and Public Protection.
