The State of Connecticut, Department of Developmental Services (DDS) is seeking proposals from DDS providers qualified to provide CRS and/or CLA supports to develop, operate, supervise and manage the administrative duties of a 3-bed transitional housing program for individuals over the age of 18 who have been diagnosed with intellectual disabilities, co-occurring mental health issues, and have experienced involvement with the criminal justice system. This facility could also be used as an interim step for individuals being discharged from a locked forensic unit. Based on capacity, admission could be expanded to serve individuals who are discharged from incarceration, involved with probation and parole, homeless or anticipate homelessness, have an unsuccessful residential placement, have probate involuntary placement, have loss of caregiver due to death, illness or other loss (eviction, foreclosure), unnecessary emergency admission, or unnecessary hospitalization.
The State of Connecticut, Department of Developmental Services (DDS) is seeking proposals from DDS qualified providers to provide short term emergency transitional housing for individuals with intellectual disabilities, ages 18 and over, who are homeless, at imminent risk of homelessness, and/or being discharged from the criminal justice system. The program will be in state owned facility in Meriden. The successful proposer will be responsible for providing support for behavioral, mental health, and legal needs to individuals of the Department. They will maintain the health and safety of the residents, safeguard the individual funds of the residents, and follow the policies and procedures of DDS. Please be advised that the successful proposer will be required to assist program residents to secure permanent housing and/or residential supports within 180 days. Extensions to this time frame may be granted by the Regional Director. DDS will work with the provider to establish clear criteria for timely admissions and discharges from the program.

The State of Connecticut, Department of Developmental Services (DDS) is seeking proposals from organizations who have experience working with school-aged children and young adults (Ages 8-20) who have intellectual disability, to develop activities such as: supporting individuals in established community-based afterschool programs, social groups, hourly weekend respite, and independent living skill building. The activities are encouraged to be integrated within the community and locally accessible throughout Connecticut.
Many individuals with intellectual disability experience isolation and other restrictive factors that limit their ability to participated in community-based activities. Families struggle to identify appropriate afterschool programs, hourly group respite, and skill building groups for their child with intellectual disability. Proposed activities should focus on building community connections, friendships, skills and provide the family relief from the ongoing care of their child. The activities are encouraged to be integrated within the community and locally accessible throughout Connecticut. Multiple proposals will be awarded to ensure statewide coverage.
The award of any contract pursuant to this RFP is dependent upon the availability of funding to the Department. The total award amount will be divided among selected proposals based on their proposed cost. The number of awards will depend on number of submitted proposals that meet criteria to ensure statewide coverage. The contract term is anticipated to be 2 years.

DDS is seeking proposals from private providers to develop, operate, supervise and manage the administrative duties associated with the operation of this program. The successful proposer will be required to provide supports within the family home, complete a functional behavioral assessment (FBA), develop a positive behavior program, train the family on implementing the plan, model program implementation and provide consultation to other in-home support providers, and develop a transition to discharge. They will also be required to provide nursing consultation, if needed, regarding medication and medical needs.

Contracts shall be issued with at least one (1) selected vendor for fiscal intermediary services.
