What is Uniform Licensing?
Answer: DCF, DDS and DPH have come together to streamline the licensing process for thirteen license types. We are utilizing the DAS-BIZNET system to collect and store license applications and other required licensing documentation. There is also a new Uniform License Application form. The same application form will be used for the thirteen included license types.

Which licenses will be included?
Answer:

Can I still hand in paper copies of my application?
Answer: The goal for the Uniform Licensing process is for all applications to be submitted electronically. The state agencies recognize that this may be challenging at first, but the intent is for all applications and most documents to be filed online.

Will all of my documents need to be provided electronically?
Answer: No, state licensing agencies may still require some documents to be provided in a paper format. Please contact the licensing staff directly with questions regarding license requirements.

My site is licensed by more than one state agency. Do I complete all my licensing applications at the same time?
Answer: Each site will need its own license application. If that site is licensed by more than one agency, submit an application form for each license that is due, at the time of that particular license period.

How will my licensing state agency know that I have posted my documents?
Answer: State licensing agencies will receive daily email notifications informing them of changes to online licensing documents. Please continue to contact licensing staff directly for all other matters.

Can I make changes to my documents in between licensing cycles?
Answer: Yes, but please note changes to universal documents will change that document for all licensing sites, not solely for the site where the new document is uploaded.

Will any other license types be included in the future?
Answer: Additional licenses may be included in the future, depending on the success of the process with the currently chosen licenses.

If DCF, DDS, and DPH all utilize BIZNET to review licensing documents, can all three agencies view my uploaded documents?
Answer: Only the agencies you have selected for the licensing process on the BIZNET site will have access to your documents.

The directions on the document upload page for “Certificate of Incorporation” and “Certificate of Authority” tell me to post on Secretary of State CONCORD SYSTEM. Is this a new requirement?
Answer: No, if your company is already registered with the Secretary of State’s Office, you need not take further action. If you are not, visit the Secretary of State’s website and complete the documents there.

My organization is only licensed by a single state agency; does the uniform licensing apply to us?
Answer: Yes, the uniform licensing process applies to all providers licensed by one or more of the 13 included license types, regardless of the number of state agencies you are licensed with.

Who can sign the application?
Answer:
- DDS: The administrator of your agency.
- DPH: The President or Secretary of your Corporation, or Municipal Officer.
- DCF: The administrator of your agency.

Answer:
- DDS: The person responsible for the overall management, operation and provision of services within the licensed residence.
- DCF: The chief administrative officer of the agency regardless of their title.

Are electronic signatures allowable?
Answer: Yes, all three agencies allow electronic signatures.

Which agencies require the application to be notarized?
Answer: The Department of Developmental Services and the Department of Public Health require the application to be signed by a notary.

Do the notaries need to stamp their seal on the application?
Answer: No, notaries may sign the application electronically. There is no need to print out the application, stamp it, and upload it; it can be done entirely electronically.

Can I re-use a uniform licensing application for another license so I don’t have to retype all the information?
Answer: Yes, but be sure that all the information on the application is up to date, and that it is specific to the license you are applying for.

Do I have to fill out the application and upload my licensing documents all at once?
Answer: No. BIZNET will automatically save all documents you have uploaded to the site. It can be returned to as many times as necessary for completion.

I’m not sure if my company already has a BIZNET account. What should I do?
Answer: First create an account for yourself (directions here). Next fill in the “Company Information” page and hit continue. BIZNET automatically searches its database using the Federal Employer Identification Number (FEIN). If a match is found, BIZNET will provide you with the contact information of the user associated with the company account. Contact that user and have her/him add you as a new user.

I am registered with BIZNET as a contractor already. Do I need to register as a community-based entity?
Answer: If you are already a BIZNET user, the type of business your company identifies as in the “Company Information” page will not impact the Uniform Licensing process. You do not have to select Non-Profit unless that best describes your company.

BIZNET will not let me continue because I have not selected an “Industry” or an “Industry Sub-Class” for my company.
Answer: The “Industry” section of the “Company Information” page is not relevant to the Uniform Licensing process. It is only necessary for procurement. If you are using BIZNET for Uniform Licensing only, skip to the next section by selecting “Click here to Upload Documents.”

On the “Uniform Licensing Upload Page,” what is the difference between “Upload” and “Upload List”?
Answer: “Upload” is used when a licensing requirement needs a single document to be provided. “Upload List” is used when multiple documents are needed to fulfill a requirement.

How will I know that my documents are posted on the site?
Answer: If the requirement needs only a single document, the document will appear in the “Uploaded Info” column. If several documents are needed, click “Upload List” to the left of the document name. The documents will appear there, sorted alphabetically by the “Description” box.

Which documents are universal?
Answer:
Universal documents are used for all licensing sites and do not vary by site. Changes to a universal document on one site page will change that document for all other sites.
Universal documents include:
- Audit
- Board of Directors
- Bylaws

Which documents are not universal?
Answer: Documents that are referred to as “site specific” in the document description are not universal.

Describe the “audit trail” in the system.
Answer: All documents uploaded to the BIZNET system will be permanently saved by the Department of Administrative Services. DAS is able to retrieve deleted documents. When a document is deleted, the time and date of its deletion, as well as the email address of the user that deleted the document, is recorded.

What happens if I delete a document by mistake?
Answer: If you delete a document by mistake, inform your licensing contact at DDS, DCF, or DDS. They can assist you in retrieving the document.

How do I delete a document?
Answer: Click the trash can icon next to the uploaded document.
