Initial Application for CLA eLicense Online
CLA INITIAL APPLICATION
All initial applications will be completed on line through the eLicense system. All CLA developments are awarded through our RFP process and you must be a DDS Qualified Provider. Please contact the DDS.QSI@ct.gov mailbox to request an initial application.
TO COMPLETE AN INITIAL APPLICATION ONLINE
You will receive an email notice to complete an online application using the eLicense website. This notice will contain a temporary username and temporary password you will need to log into the eLicense website.
Once you have received the Initial Application notice email containing your temporary userid and temporary password, proceed to the online initial application website by clicking the link below:

If you did not receive a notice containing your temporary userid and temporary password, send an email to DDS.eLicensing@ct.gov and include the following:
- Name
- Agency
- Program Name
- Email address
-Indicate that you are requesting an Initial Application userid and password access to complete your initial application only.
Please note we cannot provide the userid and password over the telephone.
