Update Provider Licensee Information
Online (CLA ONLY):
- Go to www.elicense.ct.gov
- Access your account by entering your User Id and Password
- Select Address and General Maintenance under Online Services to submit your address change
- If you need your User Id and Password, submit a request to DDS.eLicensing@ct.gov
Email (CCH* & CLA):
To report a change of address, send an email to DDS.eLicensing@ct.gov to include the following:
- CCH* Licensee Name OR CLA Program and Agency Name
- License Type (CCH* or CLA)
- License Number
- Email address
- Current Address on record
- New address
*NOTE: Community Companion HOme (CCH) was previously referred to as Community Training Home (CTH)
To make changes to existing Provider Profile, email the completed form(s) to DDS.Provider.Profiles@ct.gov
