Health and Clinical Services Forms
Documentation is an important means of confirming and communicating care delivery. One mechanism of streamlining communication is to establish basic standard for important communication. The Department of Developmental Services ’ Health Services Division has made available some of the DDS forms solely to facilitate ease of access and documentation consistency.
The forms on this page are the DDS approved forms, they are not required to be used exactly as outlined. However, for purposes of consistency, we recommend the use of these or similar forms. Providers may modify forms by adding additional information and/or reconfiguring the outline of the form. It is critical however, that the modified form contain similar information to meet the intent of the DDS approved forms. Modified forms should be sent to your regional health services director. for review by the statewide health services team.
