Health and Clinical Services Forms

Documentation is an important means of confirming and communicating care delivery. One mechanism of streamlining communication is to establish basic standard for important communication. The Department of Developmental Services’ Health Services Division has made available some of the DDS forms solely to facilitate ease of access and documentation consistency.

The forms on this page are the DDS approved forms. For purposes of consistency, we require the use of these forms. Providers may modify the Consultation Appointment Record and Seizure Record forms only by adding additional information and/or reconfiguring the outline of the form. Those forms when modified don’t require review. Providers may reach out to the regional nurse consultant for assistance if needed. It is critical however, that the modified form meet the intent of the DDS approved forms.
