Activating My S-EBT Card
- Call 1-888-328-2666 - the phone number on the back of the card.
- Select 1 for English or 2 for Spanish.
- Enter the 18-digit card number on the front of the card.
- Enter the Date of Birth of the person whose name is on the front of the card using 2-digit for month, 2-digit for day, and 4-digit for year.
- Example: October 3, 1998 = 10031998
- If the card is in the name of a parent/guardian, enter the last four digits of their Social Security Number (SSN).
- If the person whose name is on the front of the card does not have an SSN, enter four zeroes (0000) in place of the SSN.
- If the card is in your child's name, enter four zeroes (0000) in place of the SSN.
- Choose a 4-digit PIN.
- Re-enter the 4-digit PIN.

To use your S-EBT card, you must enter your 4-digit PIN into the store card reader. For online purchases, enter your 4-digit PIN when prompted.
You must protect the S-EBT card and PIN.
- DO NOT WRITE YOUR PIN on the card
- DO NOT let anyone know your PIN, and
- Change your PIN often
Lost or stolen benefits will not be replaced. If S-EBT benefits are taken from a lost or stolen card, we can replace the card, but not the benefits. If any S-EBT benefits are left on your old card, it will automatically transfer to your new card.
We cannot replace S-EBT benefits that are stolen electronically, due to card cloning or skimming.
If your household experiences a misfortune or a disaster, such as a fire or flood, the benefits may be replaceable. To report a household misfortune, contact the DSS Benefit Center at 1-855-626-6632 or go to a local DSS Resource Center.
To report a card as lost or stolen, you should immediately call 1-888-328-2666.
SAVE YOUR CARD! EACH YEAR, S-EBT BENEFITS WILL GO ONTO THE SAME CARD!
