The Division of Contract Management (DCM) can help you with contract clarity, scope, Personal Service Agreement (PSA) writing, and more.

Contracting agencies decide on cost and service. Agencies follow statutes if they’re required to pick a contractor through a Request for Proposal (RFP).

Phase 2: Requests for Proposals (RFPs)

If required, the agency holds an RFP review to pick a contractor. Learn about the six steps in a typical RFP process. Get important forms.

Purchase of Service (POS) materials

This legal document defines services or products. The agency purchases ongoing, routine client services from private providers like extended day treatment centers.

This contract defines the services or products for the agency. It’s usually used to purchase infrequent services like specialized health, technical help, and training.

DCF foster and adoptive homes are licensed by the Foster Care Division (FDC) within the DCF Area Office. For information, call 1-888-KID-HERO (1-888-543-4376) or click on the Foster and Adoptive Licenses link below. Child day care homes and day care centers are licensed by the Office of Early Childhood (OEC). To learn about becoming a licensed day care provider, visit the OEC website.

DCF-823 (PDF) 

Exceptional Circumstances form. For critical incidents and significant events to a child. Mostly used by internal workers and facilities.

DCF-465 (PDF)  

Psychotropic Medication Consent Requests. Medication change form for children used by hospitals.

DCF 460 (PDF) 

Informed Consent for Necessary or Emergency Health Care. For emergency children’s surgeries, used by hospitals.

The PIE System was created for the Department of Children and Families (DCF) in Connecticut. It helps providers track the care of individuals and report on service outcomes for state reporting requirements, as well as grant-writing purposes. PIE further allows DCF to aggregate the data and report on Quality of Life outcomes across the state.
