Definition
PREFERRED PROVIDER NETWORK – A network of preferred providers within a managed care organization.

Fees
Initial: $2,750.00
Reinstatement: $2,750.00
Renewal: $2,750.00
No pro-rating, non-refundable application fee.
Duration
One (1) year – Renews June 30th EVERY year.
How to Apply
- NIPR online application: Apply by going to the National Insurance Producer Registry (NIPR). Under Licensing Center, select “Apply for a New License.”
- Complete the Preferred Provider Network License Application supplemental form with the required supporting documents.
How to Renew
- Renewal notices are EMAILED about 90 days before the expiration date. To verify/update the email address the Department has on file, go to NIPR’s Change Contact Information.
- NIPR online application: Apply on the National Insurance Producer Registry (NIPR). Under Licensing Center, select “Renew an Existing License.”
- Complete the Preferred Provider Network License Application supplemental form with the required supporting documents.
