CRIMINAL HISTORY REQUEST PROCESS
Individual renewal applicants must submit to a Name/Date of Birth background criminal history check with the Department of Emergency Services and Public Protection – Division of the State Police (DESPP). DESPP will submit background results directly to the Insurance Department for review. Applicants are responsible for initiating the background process with the State Police in a timely manner. The Insurance Department is not responsible for background results received after the renewal deadline.

PAPER CRIMINAL HISTORY REQUEST FORMS WILL NO LONGER BE ACCEPTED BY DESPP.
Please go to the following online portal link to submit your criminal history request. In order for the results to be appropriately returned to the Insurance Department, the 8-digit agency service code must be entered to the correct portal information box which is called: Agency Provided Service Code – Name and Date of Birth

The agency service code is included in your renewal/assessment notification email. If you have not received an email, please contact the Department at the email address below.
The agency service code above must be used to request your criminal history. If a request is submitted without using the service code, the results will go to the individual. These results will NOT be accepted by the Department.
Please submit the criminal history request in a timely manner as the Insurance Department will not be responsible for any background results received after the renewal deadline.
