File Changes for Registered RPGs
If you are a Risk Purchasing Group domiciled in a state other than Connecticut and would like to register to do business in Connecticut, please review the following information and fill out Form 1 and Form 2:
- Form 1 – NAIC Risk Purchasing Group Part A and B Application Form
- Form 2 – Acknowledgement Receipt of Bulletin No. 21
- Licensing Requirements for Persons Involved with Motor Vehicle Servicing Agreements
- If the RPG is organized as a Connecticut domestic entity, submit a copy of the RPG’s Certificate of Incorporation, accepted or certified by the Connecticut Secretary of the State's office.

No registration fees or annual fees are applicable to RRGs registered in Connecticut.

If you are a Risk Retention Group domiciled in a state other than Connecticut and would like to register to do business in Connecticut, please fill out the following forms if applicable:
- Form 1 – NAIC Uniform Risk Retention Group Notice & Registration Form
- Form 2 – Acknowledgement Receipt of Bulletin No. 21
If you are writing medical malpractice liability coverage, please fill out this form:
Please also include a copy of the RRG’s following information if applicable:
- Certification of Authority
- Business Plan
- Feasibility Study
- Last quarterly financial statements
- Last annual audited financial statements
- Last actuarial opinion
- Last examination report
- Reinsurance agreement
