CERTIFICATES OF INSURANCE
It has come to the attention of this Department that in many cases certificates of insurance are not being furnished to all insureds under group, franchise, or blanket type policies.
We ask that you immediately file certificates, in the event that you have not previously done so, in connection with all group life and accident and health filings and in addition with respect to all blanket type coverages such as student accident, volunteer firemen, team coverages and other similar plans.
As of this date, certificates will be required to be filed with all submissions of policy forms of this type. March 22, 1965
