Reporting Immunization Information

CT WiZ Reporting is Mandated as of July 1, 2022 

Health care providers who administer immunizations in Connecticut must report electronically to CT WiZ.

Promoting Interoperability 

Providers and clinics who do not administer immunizations do not report to CT WiZ. Use the current project status letter to certify that you are attesting to Promoting Interoperability.

Application For Electronic Reporting to CT WiZ 

All vaccinating health care providers are required to report all vaccinations for all ages electronically to CT WiZ. To submit your intention to start the CT WiZ electronic reporting process, click the link above to complete the application.
