Once your clinic is reporting electronically to CT WiZ (by Electronic Health Record or the User Interface), your clinic must post/provide the:
- Notice to Patients About Reporting to CT WiZ (English-Spanish).
- The notice states that immunizations given to patients are reported to CT WiZ. In addition, Connecticut hospital birth packets and home birth packets include this notice so new parents know their children are automatically enrolled in CT WiZ.
