Access Your Immunization Record

The CT WiZ Public Portal allows individuals who were vaccinated in Connecticut to access their own immunization records, while parents and guardians can also access their minor child’s records. You have the option of getting a PDF copy of your full immunization record, or your COVID-19 vaccine record that contains your SMART Health card.

Complete the online form below if your (or your minor child's) immunization record is missing immunizations or was not found in the CT WiZ Public Portal. 

  • CT WiZ began collecting records for young children statewide in 1998.
  • CT WiZ expanded to collect records for all residents of all ages starting in 2022.

Note: Historical doses may not be in CT WiZ.
