Paramedic Licensure by Endorsement of an Out-of-State License

Before applying for certification, please familiarize yourself with the general certification/licensure policies.

In order to be eligible for paramedic licensure by endorsement, an applicant shall either:
  • Hold a current, valid paramedic license in a state or territory which maintains licensing standards at least equal to Connecticut's; or
  • Paramedics holding a current license issued by a state or territory of the United States that does not require successful completion of the NREMT examinations must obtain NREMT certification prior to the completion of the paramedic application; or
  • Hold a current paramedic license or certification in at least one other jurisdiction in the United States and has practiced for not less than four years.
  • Please note that there will be a question on the application that you will answer regarding 4 years of licensed work experience. So long as you answer this question affirmatively, your application will be processed based on the work experience. Note that you will not be required to hold NREMT certification
  • Successful completion of a Mental Health First Aid training program by a National Council for Behavioral Health (NCBH) certified instructor or a program approved by the Connecticut Department of Public Health.

The Office of Emergency Medical Services will review this information and send written notification to you regarding your eligibility for licensure by endorsement.  It is the responsibility of the applicant to check the status of the application.

Documentation Requirements

Applicants must submit the following to the Office of Emergency Medical Services:

  • A completed online Connecticut paramedic application and fee in the amount of $150.00.  Applications are only being accepted online.  Paper applications are not accepted.  Please select this link to apply online.   • Official verification of all paramedic licenses ever held by the applicant.  Section one of the Verification of Licensure form must be completed by applicant.  Section Two must be completed by each state or territory in which the applicant is or ever has been licensed.  Completed form must be sent directly to the Office of Emergency Medical Services by the verifying state.  Contact each state or territory for completion fee information.  The state contact information can be accessed on the NREMT website. 
  • NREMT paramedic certification  Please note: Due to the online certification verification by the NREMT, this office can perform verification of registry certifications.  If applicable, please ensure that your NREMT certification number is indicated on the application.
  • Certification of successful completion of a Mental Health First Aid training program by a National Council for Behavioral Health (NCBH) certified instructor or a program approved by the Connecticut Department of Public Health.
