Health Care Practitioner Renewal Information
Renewal Notices
Most licensees can expect to receive renewal notification approximately 60 days prior to expiration and if not renewed, a final notice 30 days following expiration, at the licensee's address of record on file in this office. Please keep your address current.
Renewal notices sent to licensees with expiration dates on and after June 30, 2019, include information on how the license can be renewed online. If you wish to renew your license online or have not received a renewal notice, please select this link for instructions.
Licensees who have not received a renewal notification within these time frames and do not wish to renew online should email us and request that a renewal application be mailed.
Physicians, dentists, nurses, physician assistants, master's level social workers, genetic counselors and tattoo technicians receive renewal notifications by email only.

Renewal Fee Waiver - Licensees on Active Duty in U.S. Armed Forces
Licensees who are on active duty in the Armed Forces of the United States may request exemption from the license renewal fee provided they are also a legal resident of Connecticut.
In order to qualify for exemption, the following must be submitted along with the completed and signed renewal application:
A letter from the commanding officer attesting that the licensee is currently on active duty; and
Proof of legal Connecticut residence in the form of a copy of a recent voter registration card or a letter from the town of residence attesting that the licensee is a resident.
You may submit these items directly to the office via email to oplc.dph@ct.gov or you may send them via mail to:
Department of Public Health
Renewal Fee Waiver-Active Duty Armed Forces
410 Capitol Avenue, MS #12MQA
Hartford, CT 06134-0308
Please note that fee waiver must be requested each time the license is renewed. If you do not meet the criteria above, you are required to pay the renewal fee.
Inactive Status
There is no inactive status for licenses; a licensee would either need to keep the license current or let it lapse due to non-renewal. Once a license has lapsed due to non-renewal and the licensee subsequently would like to reactivate the license, the licensee would need to apply for and meet the requirements for reinstatement specific to the profession.
Grace Period
Pursuant to Section 19a-88(e)(4)f, Connecticut General Statutes, all licenses issued by the PLIS have a grace period of 90 days following expiration during which time the licensee may continue to practice and renew the license. On the 91st day, the license becomes void and the licensee must apply for reinstatement.
