Licensing Requirements

Before applying for licensure, please familiarize yourself with the general licensing policies.
An applicant for licensure must meet the eligibility requirements outlined below:
 
Two (2) years of post-secondary education which includes coursework in chemistry, physics or mathematics and biology;
 
Graduation from a program accredited by the Council on Podiatric Medical Education (CPME) of the American Podiatric Medical Association with the D.P.M. degree;
 
Successful completion of the Parts 1, !! and !! of the examinations administered by the National Board of Podiatric Medical Examiners ;
 
Documentation Requirements
 
The following supporting documents must be forwarded directly to this office from the prime source:
 
Official transcripts sent directly to this office of pre-podiatry college education;
 
Official transcripts sent directly to this office from the accredited school of podiatry verifying degree earned and date granted;
 
Official report submitted directly to this office of National Board scores;
 
Federation of Podiatric Medical Boards disciplinary inquiry;
 
If applicable, official verification(s), sent directly from each state licensing authority where you hold/held a license, certification or registration. Please contact the jurisdiction prior to the submission of the form as some may charge a fee for this service;
 
A completed application and fee in the amount of $565.  Applications are only accepted online.  Please select this link to apply online.
