Licensure Portability through the Servicemembers Civil Relief Act (SCRA)
The SCRA was established by the United States Congress to allow for servicemembers and their spouses to use their professional licenses and certificates when they relocate due to military orders.
Servicemembers and their spouses who are ordered to active duty in Connecticut may qualify for Connecticut license for the duration of the orders pursuant to the SRCA by applying for the Connecticut license and submission of supporting documents.
Documentation Requirements
A completed application. Applications are only accepted online. Please select this link to apply online. After you have registered for an account, please select 'Initial Application' at the bottom left of the screen, expand the 'Public Health Practitioners' grouping and then select 'Start' next to 'Servicemember Civil Relief Act'.
As part of the application, the applicant will upload the following documents:
A copy of the military orders assigning the applicant or the applicant's spouse to active duty in Connecticut;
If you are the spouse of an active-duty servicemember a copy of your marriage license; and
A completed, notarized affidavit attesting that the applicant has practiced in the profession within the previous two years. Note that you need not have practiced the entirety of the previous two years-practice at any time during the previous two years qualifies.
Please note that you must remain in good standing with the licensing authority that issued the covered license and every other licensing authority that issued a license or certificate valid for a similar scope of practice and in the discipline applied for in Connecticut.
You must be in compliance with any applicable Connecticut continuing education requirement.
