Licensure Based on a Council on Endorsed Licensure Mobility for Optometrists Certificate Issued by the Association of Regulatory Boards of Optometry
Before applying for licensure, please familiarize yourself with the general licensing policies.
The Department may issue a license to an applicant who is a currently licensed practicing practitoner who holds a Council on Endorsed Licensure Mobility for Optometrists Certificate Issued by the Association of Regulatory Boards of Optometry (CELMO)
Documentation Requirements
An applicant for licensure shall arrange for the submission of the following documentation directly to this office:
Verification of Council on Endorsed Licensure Mobility for Optometrists Certificate (CELMO) Issued by the Association of Regulatory Boards of Optometry;
If applicable, official verification, sent directly from each state licensing authority where the applicant holds or has held a license. Please contact the state prior submitted the form as a fee may be required
A completed application with photograph attached. Applications are only accepted online. Please select this link to apply online. The application fee is $565.
