Perfusionist Licensing Requirements
Before applying for licensure, please familiarize yourself with the general licensing policies.
An applicant may qualify for licensure by meeting the following requirements:
Has successfully completed a perfusion education program with standards established by the Accreditation Committee for Perfusion Education and approved by the Commission on Accreditation of Allied Health Education (
CAAHE);
Has completed a minimum of fifty perfusion cases after graduation from a perfusion education program;
Documentation Requirements
An applicant for licensure must arrange for the following documents to be submitted directly to this office:
An official transcript of perfusion education forwarded directly by the educational institution to this office;
Official verification of completion of a minimum of 50 perfusion cases forwarded directly from the appropriate authority to this office (please select this
link for the required form); and
If applicable, official verification of any out-of-state license, certification or registration (current or expired). Please check with the appropriate agency, as some agencies charge a fee for completion of the
form;
A completed application with photograph attached. Applications are only accepted online. Please select this link to apply online. The application fee is $315.
