COVID-19 Community Levels Update, March 24, 2023: The CDC has listed all eight Connecticut counties in the Low/Green category as part of its weekly Community Levels update. People who are at a high risk for severe illness should consider additional measures to minimize their exposure to COVID-19 and respiratory illnesses. Perfusionist Licensing Requirements

 

Before applying for licensure, please familiarize yourself with the general licensing policies.

 
An applicant may qualify for licensure by meeting the following requirements:
 
Has successfully completed a perfusion education program with standards established by the Accreditation Committee for Perfusion Education and approved by the Commission on Accreditation of Allied Health Education (CAAHE);
 
Has completed a minimum of fifty perfusion cases after graduation from a perfusion education program;
 
Has successfully completed the certification examination (Parts I and II) offered by the American Board of Cardiovascular Perfusion.
 
Documentation Requirements
 
An applicant for licensure must arrange for the following documents to be submitted directly to this office:
 
An official transcript of perfusion education forwarded directly by the educational institution to this office;
 
Official verification passage of the certification examinations sent directly to this office from the American Board of Cardiovascular Perfusion;
 
Official verification of completion of a minimum of 50 perfusion cases forwarded directly from the appropriate authority to this office (please select this link for the required form); and
 
If applicable, official verification of any out-of-state license, certification or registration (current or expired).  Please check with the appropriate agency, as some agencies charge a fee for completion of the form;
