Dental Hygienist Licensure Requirements

An applicant for licensure must meet the following eligibility requirements:

 

Graduated from a Commission on Dental Accreditation (CODA) accredited dental hygiene program;

 

Successfully complete the National Board Dental Hygiene Examination;

 

Successfully complete one of the following clinical performance examinations:

Documentation Requirements

 

Applicants should arrange for submission of the following documentation directly to this office from the source:

 

An official transcripts of dental hygienist education, verifying the award of the degree in dental hygiene;

 

An official report for National Board scores;

 

An official report of successful completion of a regional board examination as outlined above.

 

Verification of all license held, current or expired, sent from each state or territory in which you have ever held a license.  Most states charge a fee for this service.  Please contact each state or territory for fee information.
