Verification from the appropriate authority confirming the applicant's most recent employment including dates and overall evaluation of the applicant's ability to practice with reasonable skill and safety; and
If applicable, verification of any out-of-state license, certification or registration (current or expired). Please check with the appropriate agency, as some agencies charge a fee for completion of the form.
Note: Typically, applicants for reinstatement who have not been in active, clinical practice for longer than five years are required to complete a period of refresher training and examination. Applicants to whom this applies will receive further information from the Department upon receipt of all required documentation.
