Forms and fees for medical providers

Medical providers play a key role in the workers' compensation system. Here’s how you help in every stage:

  • Diagnose and treat workplace injuries and illnesses
  • Document the injury or illness, treatment plan, and the patient’s progress and work status
  • Perform Commission Medical Exams (CMEs) and Respondent Medical Exams (RMEs) when requested

WCC allows any provider who is licensed by the State of Connecticut and willing to comply with its fee schedules and guidelines to practice within the system. For assistance with licensing, contact the Department of Public Health.

For further information on the obligations and requirements of medical providers in the system, providers and facilities should review Connecticut General Statutes Sec. 31-294d as well as Regulations for Connecticut State Agencies Sections 31-279-9 (obligations of attending physicians) and 31-280-1 (standards for medical providers).

Sell workers’ compensation insurance in CT

Get licensed by the CT Insurance Department (CID)

The CID can help you get licensed to sell workers’ comp insurance in CT. They also license insurance adjusters. Visit CID’s licensing home page to learn more. It includes a list of companies with courses and materials to help producers and adjusters pass their exams. CID also sets workers’ comp rates (premiums) after consulting with the National Council on Compensation Insurance (NCCI). CID posts the rates annually on their Commissioner’s Orders & Decisions page.
