As part of this program, participating providers will receive quarterly data reports to assist in monitoring their performance on the program’s quality measures. Your second data report was delivered in early August through your practice’s Community Health Network of Connecticut, Inc.® (CHNCT) Provider Engagement Services (PES) representative.
For Performance Year (PY) 2, DSS will apply limited updates to PY 1 Case Rates based on risk adjustment and trend. To better understand your practice’s PY 2 Case Rates, DSS, CHN, and Mercer will be hosting a Provider Forum on November 12th from 12:00 p.m. - 1:00 p.m. During this meeting, we will provide an overview of the case rate update process and answer any questions you may have.
