This is a mockup of the provider supplemental file that Gainwell will post on the provider's web portal (www.ctdssmap.com) for download. The dropdown on the Trade Files panel will have 2 options related to maternity bundle:

• * Maternity Bundle Accountable
• * Maternity Bundle Payment

The Maternity Bundle Accountable file will be generated for every accountable provider (billing provider) associated with a case rate payment in the financial reporting period. The file will contain all case rate payments associated to all accountable providers associated with the practice ID in the financial reporting period. (This will include all case rate payments for all accountable providers associated with the TIN.)

This file will be available to the provider practice that is receiving the case rate payments on behalf of the TIN. If the payment provider is also an accountable provider, they will receive 2 files. The format of both files is the same.
