Choosing a Nursing Home
Choosing a nursing home for yourself or a loved one is a very important decision. Consumers should always visit each facility they are considering and spend time touring the facility and talking with residents/ families and staff members.
Step 1: Research and prepare for personal visits
- Contact the facility's admissions representative to schedule and appointment for an informational meeting and tour of each facility. Ask for a copy of the facility's brochure, admissions policies, and the resident's bill of rights.
- Contact the ombudsman who represents the facilities you are interested in
- Research the latest survey findings by going to the state licensure website and typing in the name of the facility
- Visit the Centers for Medicare and Medicaid Services to utilize the Nursing Home Compare site
Step 2: Visit each nursing home
- Meet with the Administrator and Care Staff
- Topics and questions to ask during your visit
- Tour the nursing home and observe staff interaction with residents
- Take the time to speak with residents and family members about their experiences

Step 3: Narrow your choices and make a selection
- Revisit your top choices. Try to go at different times of the day if possible.
- Contact the admissions staff about the admission process.
- Discuss specific accommodations or concerns you have
- Ask to participate in care planning decisions
- Be sure to visit often and remain involved in ongoing care and decisions
