The Statewide Payroll & Time Management Division (SPTM) has made significant changes to the On-line Check form. The form is now formatted using Microsoft Excel. Requests must be made electronically using the new format and emailed to SPTM using a dedicated email address.
To ease the transition, SPTM will accept old On-line Check Request forms until September 1, 2026. After September 1st, SPTM will return old request forms to the agency and request that the new Excel form be used. SPTM will not accept paper forms effective immediately.
Changes include:
• Updated section headings.
• Removed tax elections
o Tax election data exists in Core-CT.
o If a change is needed, the employee must file a W-4 and/or CT-W4.
Must be entered prior to online check being entered.
• Rearranged tax fields.
o Split FICA into its separate Social Security and Medicare components.
• Gross to net calculation in spreadsheet
• Removed “Deduction Authorization” and employee signature section.
• Removed wet signature requirement
o An official email from the agency authorized representative is sufficient.
o Requests from unauthorized individuals will be rejected.
To initiate an On-line Check request, complete the Excel form and save it using the following file naming convention: department ID-employee number-empl record number-check date (mm-dd-yyyy). For example, OSC15000-123456-0-05-15-2026.xlxs
