Affiliate filings
Connecticut hospitals and hospital systems must submit a written report describing each affiliation with another hospital or hospital system.
Annual reporting and 12-month actual filings
Hospitals must report annually on their actual operating results. See a complete list and description of the required annual reports.
Facility fee filings
Hospitals must report information regarding facility fees charged or billed during the preceding calendar year.
Group practice filing
Group practices of 30 or more physicians must submit an annual report of information describing each affiliation with another hospital or hospital system.
Pricemaster/chargemaster filings
Pricemasters, also known as chargemasters, are the detailed schedule of all hospital charges required to be on file with the OHS.
Specialty hospital filings
Specialty hospitals submit copies of their Audited Financial Statements five months after their fiscal year ends.
Medical foundations notifications and filings
Medical foundations must file a copy of their certificate of incorporation and any amendments.
Material change notification
Transactions resulting in a material change to a group practice require written notice to the Commissioner of Public Health within 30 days.
