The first weeks after birth can be challenging. Family Bridge is a free service for families with new babies in the Greater Bridgeport and Eastern Connecticut areas. The program is available to families who give birth at Bridgeport Hospital, St. Vincent’s medical Center, and Backus Hospital. This pilot program supports families during the transition from hospital to home by providing in-home visits from Registered Nurses (RN) and Community Health Workers (CHW).
