Group Practice Notifications
Each Connecticut hospital and hospital system, as well as each group practice comprised of thirty or more physicians (“filers”) must submit to the Office of the Attorney General (“OAG”) and to the Office of Health Strategy (“OHS”), an Annual Report of information concerning these group practices.What is the statutory authority for this filing requirement?
Connecticut General Statutes (C.G.S.) §19a-486i(g) and (h), as amended by Section 56(g) and (h) of Public Act 18-91.
When is this filing due?
The Annual Reports must be filed with OAG and OHS no later than January 15, 2018, and annually thereafter.
Who needs to submit this filing requirement?
- Connecticut hospitals as defined in C.G.S. §19a-490, which includes all general, children's, chronic disease, psychiatric, hospital and maternity hospital facilities.
- Hospital Systems as defined in C.G.S. §19a-486i.
- Each group practice in Connecticut that is not owned or affiliate with a hospital or hospital system and is comprised of 30 or more physicians. Group practice is defined in C.G.S., §19a-486i(a)(10).
Provided below is a link to a standardized excel worksheet that has been jointly created by OHS and OAG for these filings. The Excel worksheet has been provided to ensure consistent filings to OHS and OAG. Please provide the Annual Report to OHS and OAG no later than January 15th each year. A separate document with instructions for filling out the Excel file is also provided below. The completed Excel file should be uploaded to the Notifications and Filings portal which will simultaneously file the data with both the OAG and OHS. The detailed instructions for uploading are provided below.
