Reporting a complaint

There are a number of different ways to report a concern or a complaint involving a healthcare provider you may want to explore.

Connecticut Department of Public Health

You can report a complaint about a licensed healthcare provider through the Department of Public Health. You can find information on how to complete the process online or by calling the Practitioner Investigations Unit at 860-509-7552.

Patient Advocates

Most hospitals have an assigned Patient Advocate to assist you with any challenges you may have before, during or after your care.  Check the hospital’s website under Patients Rights to find the advocate’s contact information, or call the hospital’s main telephone number and ask to speak to an advocate.

You may also contact The Joint Commission, the agency that accredits hospitals to report a concern or a complaint:
