How are people notified that they are automatically included in CT WiZ?

The Connecticut Immunization Information System (IIS) called “CT WiZ” is a secure, confidential system that meets national standards for IISs and maintains complete and accurate immunization records for patients vaccinated in Connecticut. Per State law, CT WiZ is an “opt out” system. This means that individuals are automatically included in CT WiZ, unless the individual sends a signed written request to decline participation in CT WiZ including the vaccine recipient’s full legal name, date of birth, and address. The request shall be sent to the Connecticut Department of Public Health CT WiZ by first class mail or by fax to the address and fax number listed on the Department’s website. Once the request is received by the Department, CT WiZ will no longer update or make available that vaccine recipients immunization record as specified in Section 19a-7h-8 of the Regulations of Connecticut State Agencies. The majority of individuals are included, and their immunization records are available to them, their providers, and to public health when needed.

This notice is provided to parents in Connecticut hospital birth packets and home birth packets, to enrollees at the time of enrollment in Medicaid, and at vaccinating facilities and clinics in Connecticut. Per State law, this notice states that people are automatically included and immunizations given to patients’ records are reported to CT WiZ through a secure, confidential electronic interface. All information in CT WiZ is kept confidential as required by law.
