Long Term Care Investigations
Public Act 99-176 redirected the reporting to and investigation of abuse, neglect, and exploitation of nursing home residents from the Office of the Long-Term Care Ombudsman to the Department of Social Service (DSS) Office of the Commissioner. This responsibility is assigned to the Social Work Services Division, which screens all calls and written reports relating to this area. Please contact the Social Work unit at 860-424-5964 for any questions regarding this procedure. Please note that this process refers only to reports made by mandatory reporters working in long-term facilities. For mandated reporters working in long-term care facilities:
To make a report of abuse, neglect or exploitation:
- By Fax: Complete the form W-410 and fax to 860-424-5091.
- Online: Submit an online referral *NEW*
- Tutorial Video on Making an Online Referral
*Do not submit the same report multiple ways.
For non-mandated reporters, made a report to the Department of Public Health (DPH) and the Long-Term Care Ombudsman.
