About HCBS

The State of Connecticut offers many home and community-based services (HCBS) to help eligible individuals who need support to live at home or to return to community living.

Connecticut has several programs; some are state plans and others are operated under a Medicaid waiver. These programs and services include: 

  • Acquired Brain Injury (ABI) - provides a range of services to help individuals with ABI live in the community rather than in an institutional setting, like a nursing home or chronic disease hospital.

  • Assisted Living Demonstration Project - these services are available to low-and moderate-income older adults (65+) who meet eligibility requirements through the Connecticut Home Care Program for Elders (CHCPE) or the Department of Mental Health and Addiction Services home and community-based program for adults with severe and persistent psychiatric disabilities. 

  • Autism Waiver - provides a range of services to help individuals with Autism Spectrum Disorder live in the community rather than in a long-term care facility.

  • Community First Choice (CFC) - allows eligible individuals to access Personal Attendant Care and other services and supports through self-direction. 

  • Connecticut Home Care Program for Elders (CHCPE) - allows older adults to receive care services and supports at home rather than at a nursing home or other long-term care facility.

  • Katie Beckett Waiver - serves children and young adults, up to the age of 21, with a physical disability who may or may not also have a developmental disability and would normally not qualify for Medicaid due to family income. 

  • Money Follows the Person (MFP) - assists people who are Medicaid-eligible and currently living in a nursing home or other long-term care facility to transition to the community. 

  • Personal Care Attendant (PCA) Waiver - pays for the costs of a personal care attendant to help people with disabilities accomplish daily activities and live independently at home instead of in an institution such as a nursing home. 

  • Department of Mental Health and Addiction Services Mental Health Waiver - provides home and community-based services for individuals with serious psychiatric disabilities.

  • Department of Developmental Disabilities Waivers

    • Individual and Family Supports Waiver - allows individuals to receive a number of specialized services and supports and the authority to self-direct care as they desire.

    • Employment and Day Supports Waiver - provides services and supports specifically to assist individuals to find and apply for jobs and maintain employment.

    • Comprehensive Supports Waiver - provides comprehensive services and supports to people with intellectual disabilities in need of a high level of care.
