TB Disease Reporting for Local Health Departments

Suspect or confirmed tuberculosis (TB) disease is a Category 1 disease and requires immediate reporting by phone to the Department of Public Health TB Control Program.

 

Health care providers are required to send a TB Surveillance Report Form to the TB Control Program, and also to the Director of Health of the jurisdiction where the  patient resides (or the location to which the patient will be discharged).

 

After the TB Control Program receives a TB Surveillance Report Form, a copy of the TB Surveillance Report Form will be sent to the Local Health Department or District (LHD) which has jurisdiction over  the location the patient resides (or will be discharged to).

 

When the LHD receives a TB Surveillance Report Form or communication from the TB Control Program, the LHD should communicate with the health care provider and with the TB patient to collaborate on the patient’s discharge and treatment planning.
